You must have the following documents to the Nearest Region Offices for NCSC Online Registration.
- Front and back photocopy OSCA ID with 3 signatures each
- 3 Photocopies of MDR Form
- PhiHealth ID
- Other government-issued ID.

NCSC Registration Philippines Online Process
- First, visit the official website – ncsc.gov.ph
- On the homepage, click on the “Register Now” Button
- Check the box. “I agree and certify that I have read the above instructions and that I fully understand”.
- Click the Proceed Button.
- Fill in the Data Form. It is important to upload your OCSA ID and Picture.
- On the last part of the data form, under this is to certify, that ….” click all 6 Boxes”).
- Upload a Selfie
- Create a Pass Key Number (PKN)
- Click Proceed.
- Check the summary shown. If correct, click the box ” I certify that the above information is correct…” then click the “Submit Now” button.
- Congratulations! You have successfully submitted your application for Registration!
